The Beta lactamase enzymes of these bacteria are very diverse and their constantly point mutations have led to the emergence of Extended-spectrum Beta lactamases ( ESBLs). In this study, which took place in a period of eight months, after confirmation of the identity of Acinetobacter strains by bacteriological tests, the pattern of drug resistance of the isolates and the production of ESBLs by them was investigated by Agar disk diffusion method.In this study, 50 strains of Acinetobacter baumannii and 15 strains of other Acinetobacter species were isolated from patients. The highest frequency of drug resistance in Acinetobacter baumannii isolates was related to Cefipime (100%), and the lowest was related to Cefixime-Clavulanic acid (50%). Acinetobacter with Multiple Resistance Patterns are a growing problem for medical centers around the world and the most important reason for this is the unnecessary and over-consumption of drugs, especially Beta lactam antibiotics.}, keywords = {Acinetobacter baumannii,Extended-spectrum Beta lactamase,Drug resistance,Hospitalized infection}, title_fa = {بررسی فنوتیپی جدایه های اسینتوباکتر بومانی مولد بتالاکتاماز وسیع الطیف (ESBL) در مراجعه کنندگان به یک بیمارستان نظامی استان گیلان}, abstract_fa = {زمینه: Acinetobacterﮐﻮﮐﻮﺑﺎﺳﻴﻞ ﮔﺮﻡ ﻣﻨﻔﻲ ﺍﺳﺖ ﮐﻪ ﻃﻲ ﺩﻫﻪ ﮔﺬﺷﺘﻪ ﺷﻴﻮﻉ ﻋﻔﻮﻧﺖﻫﺎی ﺑﻴﻤﺎﺭﺳﺘﺎﻧﻲ ﻧﺎﺷﻲ ﺍﺯ ﺍﻳﻦ ﺑﺎﮐﺘﺮی ﺩﺭ ﺣﺎﻝ ﺍﻓﺰﺍﻳﺶ ﺑﻮﺩﻩ ﺍﺳﺖ. آنزیم های بتالاکتاماز در باکتری ها بسیار متنوع بوده و عامل مهمی در مقاومت آنتی بیوتیکی محسوب می شوند.هدف: هدف از این مطالعه شناسایی فراوانی جدایه های اسینتوباکتر مولد بتالاکتامازهای وسیع الطیف در بیمارستان نظامی خصوصا بخش مراقبت های ویژه و بررسی الگوی مقاومت آنتی بیوتیکی جدایه های این باکتری و راهکارهای کاهش خطر آن.روش کار: در این مطالعه، در یک دوره هشت ماهه، تعداد 100 نمونه از قسمت‏های مختلف بیمارستان ارتش استان گیلان گرفته و از نظر وجود اسینتوباکتر مورد بررسی قرار گرفت. پس از جداسازی اعضای جنس اسینتوباکتر مولد بتالاکتاماز از طریق تست های بیوشیمیایی متنوع، الگوی مقاومت آنتی بیوتیکی بر اساس استانداردهای CLSI-2011 و همچنین فنوتیپ هاله عدم رشد این جدایه ها مورد بررسی قرار گرفت.یافته ها:نتایج با توجه به جدول CLSI گزارش گردید: بیشترین مقاومت آنتی بیوتیکی در 50 ایزوله اسینتوباکتر بومانی به ترتیب مربوط به آنتی بیوتیک های: سفی پیم100%، سفوتاکسیم 97%، تری متوپریم80% بود همچنین هاله عدم رشد در دیسک های مقاومت سفتریاکسون – کلاولانیک و سفکسیم- کلاولانیک اسید بیش از 5 میلی متر و میزان مقاومت به ترتیب55% و 50%، بود.نتیجه گیری: سویه های A.baumanni با مقاومت چندگانه و تولید کننده بتالاکتاماز به عنوان یک مشکل روزافزون برای مراکز پزشکی در دنیا مطرح هستند و همچنین درصد شیوع این باکتری در بیمارستان ها روبه افزایش است.}, keywords_fa = {Acinetobacter baumannii,Extended-spectrum Beta lactamase,Drug resistance,Hospitalized infection}, url = {https://cell.ijbio.ir/article_1301.html}, eprint = {https://cell.ijbio.ir/article_1301_ad1d5ddfb636a44bb7f9b3dc6ce82442.pdf} }
